How do I remove a custom facility?

Background:

We have created a feature Custom Facilities to segment your billings by different outpatient locations. 

How to remove a custom facility?

Note: A custom facility can ONLY be removed if no claim has been created / submitted using that facility. 

Step 1: Top right gear icon --> Settings --> My Custom Facilities

Step 2: Click Action --> Delete
