OHIP error code – V69 – Service date invalid for SLI (Service Location Indicator)

Background

Service date recorded on claim is not valid with the SLI recorded on claim.

Solution

• Double check that the correct SLI has been selected for the diagnostic procedure being billed
• Double check the service date of the claim

Once the SLI or service date is updated, you can resubmit the claim.
