OHIP error code – V64 - Missing service location indicator (SLI)

Background

The claim is missing the SLI (service location indicator) which is assigned when a hospital diagnostic service is billed by a participating hospital physician/group.

Solution

Double check the claim to make sure the correct SLI has been entered. If blank choose the correct service location indicator from the dropdown menu, save claim and resubmit.
