Background

One of the following codes has been billed for a patient that is not registered on Health Care Connect:

• Complex Vulnerable New Patient (Q053A) 
• HCC Upgrade Patient Status (Q056A) 
• HCC Greater Than (HCC GT) Three Months (Q057A) 

Solution

Contact the Home Health Care Coordinator at your local LHIN to confirm patient is registered on Health Care Connect (HCC) .
