OHIP error code – EPF- Enrollment Date Mismatch
Service date does not match the patient's enrolment effective date. Patient may not be covered by OHIP on the date of service. You may need to invoice this patient privately.
Call the Ministry (1-800-262-6524) to confirm if the patient has valid coverage for date of service. If patient does not have valid coverage for date of service, you may proceed with creating a private invoice to submit to the patient for payment of services rendered (private invoice).
