Problem

An incorrect Dx was used on a claim.

Solution

Please verify if the diagnostic code used is appropriate for the service billed.  Some services have restrictions on the diagnostic codes that can be used. Please refer to the OHIP Schedule of Benefits for your discipline for more details and/or contact your claim assessor for advice.
