How to determine which Ministry of Health (MoH) claims processing office (district office) processes your claims

If you need to confirm which MoH Branch Office processes your claims, you can call the MoH to confirm. MoH General Inquiry Phone Line: 1-800-262-6524

There are three current offices:

Toronto Claims Services Branch
5700 Yonge Street, 5^th Floor, M2M 4K5

Hamilton
119 King Street West, 10^th Floor, L8P 4Y7

Oshawa
33 King St. West, Unit 403, L1H 1A1

Note: The Sudbury office has been amalgamated to Toronto. You need to change your setting to Toronto if your claims were previously processed in Sudbury.
