How to entering percentage premium codes

Question:

I added a premium K187A code to a claim with K198A but it shows $0 dollar amount when it should have been adding an additional 15% of K198A billed for the same patient.  How to bill this correctly?

Please the attached screenshot. When there is a percentage premium (example K187A), you must check the box of the parent code (K198A) at the bottom of the claim in order to apply the premium.
