OHIP Error Code - V28 – Invalid Hospital Number

Background

The facility number selected is no longer valid. This rejection is because the MOHLTC Master Number database has been updated recently; a new facility number may have been issued as a result of the integration of hospitals.

Solution

Correct the facility number in accordance with the service dates in the claim and resubmit. If you do not know the facility number, please see How to find your OHIP hospital code.
