EQC - Group number is not registered

Background:

New billing group is not registered with the Ministry of Health on the date of service.

Solution:

  • Confirm the billing group number selected is correct
  • Contact the Ministry of Health registration (moh@ontario.ca) to confirm your billing group has been registered properly.

If the billing group has not been registered, please fill out the form in the URL below and send it to the Ministry of Health Registration (providerregistration.moh@ontario.ca).

Your service date submitted must be equal or greater than your group effective date.
