OHIP Error Code - A34 - Duplicate service codes submitted

Background

You can not bill duplicate codes on the same date.

Solution

Review services that have the same date and either change the date of one of the services, or delete the duplicate.

If you are billing codes with time units (Counselling, Anesthesia or Surgical Assistant), collapse the codes and update the quantity accordingly. As an example:
