OHIP Error Code - V09 - Invalid referral number

Background:

The referring physician billing number provided is not valid. That is, the 6 digits do not match with MOHLTC’s records (i.e., number mistyped, used CPSO number accidentally, etc.)

Please note that the billing number may not be accurate if you searched the physician name in our database. The MDBilling.ca database is compiled from all of our users, in which the information has not been verified.

Solution:

Please contact the referring physician to confirm his/her billing number.
