VH8 - No match on DOB with HN

Background:

The patient’s date of birth provided does not match the health card number (a typical mistake is the birth date and month were swapped).

 

Solution:

Correct the DOB of the patient and resubmit the claim.

If you turn on the HCV (Health Card Validation) feature in the patient’s profile page, you can simply type in the health card number and the correct date of birth will be automatically populated.

For out-of province patients, you will need to check the hospital record or contact the patient.
