Entering Claims with Spreadsheet Template


How to use a spreadsheet template to enter OHIP claims

I've been using the MD billing spreadsheet template to upload claims. I've noticed when there are more than 4 entries in a single row for the same patient (i.e. with appropriate service code, different service date, and appropriate quantity) they are NOT uploaded as part of the claim. The additional entries are ignored, with no error message provided.   
Please note when entering claims using our spreadsheet template, the service date is limited to a maximum of 4 per row. If you want to bill more than 4 service dates per patient, you will need to add additional rows, copy the patient's information, and continue entering the services.
